Applicants must complete and submit this form for full consideration. Supporting documentation such as receipts may be included to substantiate the request. The deadline to apply for the first round of funding is August 23, 2020 at 11:59pm EDT. Review of applications will begin immediately.

• Detailed description of how COVID-19 has affected student's circumstances and created this emergent need.
• Estimated expenses demonstrating impact from COVID-19.
• Other documentation as applicable to the specific nature of the request.
